This one-day course will prepare clinical professionals to provide optimal care in an ever changing health care environment. The Population Health and Care Transformation Educational Symposium will be focused on the care of the cardiac and pulmonary patient from the perspective of clinicians, patients and caregivers. Attendees will participate in an exciting day of professional development, networking and even have an opportunity to meet with quality vendors.
• Verbalize basic understanding of the current health care environment and the quadruple aim.
• Explain the management of the cardiac patient at home and in the community.
• Describe the role of the primary care provider in managing complex pulmonary patients.
• Identify ways to improve current clinical practice and enhance patient experience at the community level.
